What Is Pti In Healthcare
Practice Transformation Initiative The Practice Transformation Initiative (PTI) is a multi-stakeholder program that builds capacity within your organization to help you move from volume-based health care services to patient-centered, high-quality services.

Accelerating improvement of a set of metrics common across Medicare and commercial and Medi-Cal payers. Converting your data into actionable reports for practices. Quarterly benchmarking with other delivery systems. Training and support for practice coaches. Practice redesign techniques, improvement skills and practice facilitation. Access to experts on practice improvement. A peer-to-peer learning network comprised of transformation leaders across California’s public and private sectors. Creating a better experience for practices, providers, and patients.

The Centers for Medicare & Medicaid Services (CMS) designed the Transforming Clinical Practice Initiative (TCPI) to help you achieve large-scale health transformation. In partnership with with the Pacific Business Group on Health and its California Quality Collaborative, we are one of 29 Practice Transformation Networks nationwide providing technical assistance and peer-level support to assist you in delivering patient-centric, efficient care.

What is PTI in nursing?

What is Practice Team Information (PTI)? – Practice Team Information (PTI) collects information from a sample of Scottish general practices about face-to-face consultations (in a surgery or the patient’s home) between patients and a member of the practice team. The practice team is currently defined for PTI purposes as

all GPs including locums and registrars (GPs in training), and practice-employed nurses and their clinical assistants (for example, phlebotomists and health care assistants) – referred to here as practice nurses for short. Some of the nurses employed by NHS Boards to work in specific practices may also record data for PTI.

PTI evolved from Continuous Morbidity Recording (CMR), a scheme that collected information on GP contacts only. CMR was first piloted in 1990 in a small number of practices in Scotland. By 1998 a large sample of practices, all using the GPASS (General Practice Administration System for Scotland) clinical IT system, were participating and the CMR dataset became recognised as a ‘national’ dataset.

  1. Recognising that members of the practice team other than GPs make a significant contribution to patient care, in 2001 ISD began a number of pilot projects in CMR practices to expand data recording to include patient contacts with practice nurses, district nurses and health visitors.
  2. This culminated in April 2003 in the original GP-based CMR being formally superseded by PTI.

Since 2003 the organisation of the health service, particularly in primary care, has been in constant change. As a result, district nurses and health visitors (collectively referred to as community nurses) found it increasingly difficult to record their patient contacts for PTI, in particular because they increasingly work across a geographical area and, unlike practices nurses, are not attached to just one or two practices.

  1. A number of practices stopped recording district nursing and/or health visitor data during 2006/07 and others considered leaving PTI due to the disproportionate amount of resource required to sustain this part of the data collection.
  2. To safeguard the survival of PTI, the decision was taken in 2007 to stop recording community nursing data and focus on GP and practice nurse data only.

This means that there are now three years of data available on the wider practice team (2003/04 to 2005/06 inclusive) and nine years of data on the more narrowly defined practice team (GPs and practice nurses only) (2003/04 to the latest year, 2011/12).

Another major change over recent years has been a shift away from the use of GPASS (NHS Scotland’s national clinical IT system for general practices) towards other, commercial clinical IT systems. All practices in Scotland, including those participating in PTI, were required to move away from GPASS to using InPS-Vision or EMIS systems by the end of March 2012.

Data from non-GPASS systems were used for the first time for the publication of PTI figures on 31st March 2009, when data from nine practices using Vision were included. Of the 59 PTI practices that had complete GP and practice nurse data for the financial year 2011/12 and hence were used to calculate national estimates, 17 moved to EMIS and 3 to Vision within that year so that at 31 March 2012 19 practices were using InPS-Vision and 40 were using EMIS.

The 60 practices contained in the 2012/13 PTI sample for the financial year April 2012 to March 2013 cover about 6 per cent of the Scottish population. The total population in the practice sample is a reasonable reflection of the Scottish population with regards to age, gender and deprivation, and small imbalances due to these factors are addressed during data analysis through a process of direct standardisation (see the Statistical notes page for more details).

PTI data can be used to estimate numbers of patients consulting and the number of consultations in Scotland, shown for individual years or as trends over time, overall or broken down by gender, age or deprivation, for any condition presenting to general practices in Scotland (subject to the limitations of clinical coding and/or of small numbers for some conditions).

The data can also be used to determine the most commonly seen conditions/diseases. PTI data are frequently sought to get an indication of population prevalence of a particular condition. However, PTI can only provide estimates of ‘prevalence’ of patients consulting practice staff at least once during the year rather than actual population prevalence (this is discussed in more detail in the section on Uses and limitations of the data ).

If a wider range of staff disciplines is included, the estimated number of patients consulting is likely to be closer to the number of patients suffering from a condition (at least if some patients are seen exclusively by the additional disciplines).

  1. More information on this issue can be found on the page Impact of inclusion of more staff disciplines on patient counts.
  2. As of September 2013, PTI data is no longer collected.2012/13 is the last year for which ISD will be able to publish annual PTI data.
  3. A replacement system is under development, which will collect more detailed data from a greater number of practices and this project is expected to be operational by late 2014.

What is a prothrombin time test with an INR (PT/INR)? – A prothrombin time (PT) test measures how long it takes for a clot to form in a blood sample. An INR (international normalized ratio) is a type of calculation based on PT test results. Prothrombin is a protein made by the liver.

It is one of several substances known as clotting (coagulation) factors. When you get a cut or other injury that causes bleeding, your clotting factors work together to form a blood clot. How fast your blood clots depends on the amount of clotting factors in your blood and whether they’re working correctly.

If your blood clots too slowly, you may bleed too much after an injury. If your blood clots too fast, dangerous clots may form in your arteries or veins. A PT/INR test helps diagnose the cause of bleeding or clotting disorders. It also checks to see if a medicine that prevents blood clots is working the way it should.

Prothrombin time (PT) is a blood test that measures the time it takes for the liquid portion (plasma) of your blood to clot. A related blood test is partial thromboplastin time (PTT), A blood sample is needed, If you are taking blood-thinning medicines, you will be watched for signs of bleeding. Certain medicines can change blood test results.

Your health care provider will tell you if you need to stop taking any medicines before you have this test. This may include aspirin, heparin, antihistamines, and vitamin C.DO NOT stop or change your medicines without talking to your provider first.

Also, tell your provider if you’re taking any herbal remedies. When the needle is inserted to draw blood, some people feel moderate pain. Others feel only a prick or stinging. Afterward, there may be some throbbing or slight bruising. This soon goes away.

PT is measured in seconds. Most of the time, results are given as what is called INR (international normalized ratio). If you are not taking blood thinning medicines, such as warfarin, the normal range for your PT results is:

11 to 13.5 secondsINR of 0.8 to 1.1

If you are taking warfarin to prevent blood clots, your provider will most likely choose to keep your INR between 2.0 and 3.0. Ask your provider what result is right for you. Normal value ranges may vary slightly among different laboratories. Some labs use different measurements or test different samples.

Bleeding disorders, a group of conditions in which there is a problem with the body’s blood clotting process.Disorder in which the proteins that control blood clotting become over active ( disseminated intravascular coagulation ).Liver disease.Low level of vitamin K,

If you are taking warfarin to prevent clots, your provider will most likely choose to keep your INR between 2.0 and 3.0:

Depending on why you are taking the blood thinner, the desired level may be different.Even when your INR stays between 2.0 and 3.0, you are more likely to have bleeding problems.INR results higher than 3.0 may put you at even higher risk for bleeding.INR results lower than 2.0 may put you at risk for developing a blood clot.

A PT result that is too high or too low in someone who is taking warfarin (Coumadin) may be due to:

The wrong dose of medicineDrinking alcoholTaking certain over-the-counter (OTC) medicines, vitamins, supplements, cold medicines, antibiotics, or other medicinesEating food that changes the way the blood-thinning medicine works in your body

Your provider will teach you about taking warfarin (Coumadin) the proper way. There is little risk involved with having your blood taken. Veins and arteries vary in size from one person to another, and from one side of the body to the other. Obtaining a blood sample from some people may be more difficult than from others.

Fainting or feeling lightheadedHematoma (blood accumulating under the skin)Multiple punctures to locate veinsInfection (a slight risk any time the skin is broken)

This test is often done on people who may have bleeding problems. Their risk of bleeding is slightly higher than for people without bleeding problems. PT; Pro-time; Anticoagulant-prothrombin time; Clotting time: protime; INR; International normalized ratio Chernecky CC, Berger BJ.

What does PTN mean in nutrition?

Overview – Parenteral nutrition, often called total parenteral nutrition, is the medical term for infusing a specialized form of food through a vein (intravenously). The goal of the treatment is to correct or prevent malnutrition. Parenteral nutrition provides liquid nutrients, including carbohydrates, proteins, fats, vitamins, minerals and electrolytes.

  • Some people use parenteral nutrition to supplement feeding through a tube placed into the stomach or small bowel (enteral nutrition), and others use it by itself.
  • People whose digestive systems either can’t absorb or can’t tolerate adequate food eaten by mouth use parenteral nutrition.
  • When used outside the hospital, intravenous feeding is called home parenteral nutrition.

Using home parenteral nutrition may be necessary for weeks or months, or in some cases for life. Mayo Clinic’s approach

What is high PTI?

As INR – This ratio — which allows for easier comparisons of test results from different laboratories — is used if you take blood-thinning medications. In healthy people an INR of 1.1 or below is considered normal. An INR range of 2.0 to 3.0 is generally an effective therapeutic range for people taking warfarin for certain disorders.

What is the principle of PTI test?

Why would someone taking warfarin have high PT INR results? – Several things can affect prothrombin time test results for people taking warfarin. You may have high PT/IR results if:

You haven’t been taking the proper dose of warfarin. You’ve taken over-the-counter medicines, such cold medicines, or vitamin supplements, that can interact with warfarin. You’ve consumed food and drinks that can interact with warfarin, such as kale, spinach, cranberry juice and alcohol.

A note from Cleveland Clinic If you’re injured and bleeding, your body races to form blood clots to stop the bleeding so you can begin to heal. When that doesn’t happen, healthcare providers use a prothrombin time (PT) test to find out why. You may have regular PT tests because you’re taking the blood thinner warfarin (Coumadin).

What is normal PTI in percentage?

Normal values include the following : The reference range for prothrombin time is 11.0-12.5 seconds; 85%-100% (although the normal range depends on reagents used for PT) Full anticoagulant therapy: >1.5-2 times control value; 20%-30%

What does INP mean in medical terms?

Professional (PRO) Independent Nurse Provider (INP) is an organization that is designed to allow nurses (LVN’s and RN’s) become independent. What does being independent mean? Patients who have ongoing skilled care, i.e. tracheotomy care, gastronomy care, IV administration, stoma care, qualify for EPDST (Early and Periodic Diagnosis, Screening and Treatment) through their Medicaid benefits to receive shift care or private duty nursing up to the age of 21.

  • Adult patients may qualify for the same services through the In Home Operations benefit with Medi-Cal.
  • California Children’s Services may also provide this benefit for those members who qualify.
  • Currently the majority of this benefit is administered through a Home Health Agency that supplies the nurses for the patient.

However, becoming an Independent Nurse Provider allows you to work independently without an agency and bill Medi-Cal directly for the services rendered to your patient. Our team currently consists of a Registered Nurse who has been providing private duty nursing to both adults and pediatrics for over 8 years.

  • Our Registered Nurse, Andre Bagramyan ( NPI Number 1841698479 ), started off as a LVN in 2006.
  • Having no experience at all in the nursing world and unable to find work in an acute setting, he turned to private duty nursing.
  • Unfortunately the pay was too low and Andre returned back to school to obtain his license as a Registered Nurse.

Through the first year of his RN career, he began hopping around in different areas of nursing practice to find his “nitch”. If you are still unsure whether the academic you are considering is trustworthy, you can always check their customer reviews. Most of these reviews give high ratings for the service.

  • You can also check the website’s quality assurance policy.
  • This policy ensures that you are getting the best possible work.
  • You can also choose from their loyalty program, where you can receive discounts if you order ten or more papers.
  • Unfortunately, through many position and different settings, he realized that private duty nursing and home health was where he belonged.

He again returned to school to obtain his Bachelor’s Degree in order to become the Director of Nursing and Administrator for a local Home Heath Agency. After putting in long hours and average pay, Andre turned to Medi-Cal and developed a Independent Nurse Provider relationship with them.

  • He now assists nurses in becoming Independent Nurse Provider in California to both, medically fragile foster children and private duty nurses.
  • Michelle Salazar, our Medical Biller, has been providing billing services in California for Home Health Agencies that provide Private Duty Nursing for over 10 years.

She has experience billing Medicare, Medicaid, Private Insurances and the VA. She has dealt with the most complicated billing cases and has claimed hundreds of thousands of dollars in collections. She is a great communicator with customer service to our clients as her number one priority.
