The top seven companies operating pharmacies—CVS Health, Walgreens Boots Alliance, Cigna, UnitedHealth Group, Walmart, Kroger, and Rite Aid—accounted for 70% of U.S. prescription dispensing revenues in 2022. The top 15 pharmacies accounted for more than 75% of total dispensing revenues from retail, mail, long-term care, and specialty pharmacies.

The total share of the largest 15 companies decreased slightly compared with their 2021 share, due primarily to the significant revenue losses for Walgreens Boots Alliance’s AllianceRx Walgreens Pharmacy (ARxWP). As I had predicted in 2020, the meltdown of the Prime Therapeutics/Walgreens relationship meant that most of Prime’s plan sponsor clients have shifted specialty fulfillment to Express Scripts. These lost clients reduced AllianceRx Walgreens Prime’s revenues by $8 billion in 2022. We discuss other market changes and acquisitions that affected the 2022 revenues and growth rates for the largest companies in the following sections of our new report: Section 2.3.3., Section 3.3.2., Section 3.3.4, and Section 5.2.2.

Consistent with prior years’ analysis, five of the largest U.S. pharmacies were central fill mail and specialty pharmacies owned by vertically integrated organizations that also own a PBM: Caremark (CVS Health), Express Scripts (Cigna), Envolve Health (Centene), CenterWell (Humana), and OptumRx (UnitedHealth Group). Revenue growth at the PBMs’ pharmacies is being driven by the dispensing of more-expensive specialty medications, which accounted for nearly 40% of the pharmacy industry’s prescription revenues in 2022. In the report’s Section 12.3.1., we explain how PBMs have combined into vertically integrated organizations that offer health insurance, operate specialty pharmacies, and provide healthcare services. The revenue figures in the chart above reflect combined dispensing revenues from all entities within these organizations. The matrix below summarizes the various roles that these companies play in the drug channel. Retail pharmacies have played a crucial role in the administration of COVID-19 vaccines. Pharmacies’ revenues and profits have benefited from this activity, although this advantage diminished significantly in 2022. For retail pharmacies, we estimate that total revenues from administering COVID-19 vaccines were more than $7.5 billion in 2021 and about $3.6 billion in 2022. To permit comparability with previous years’ figures, the prescription dispensing figures above exclude revenues from the administration of COVID-19 vaccines. Since pharmacies incurred no cost of goods for COVID-19 vaccines, pharmacies’ gross margins for administering COVID-19 vaccines have been 100%. Pharmacies’ operating profits from the COVID-19 vaccines were about 50% of revenues. These profits provided economic support for many retail pharmacies and contributed to fewer acquisitions in 2022.

Various announced mergers and acquisitions will alter the largest 15 pharmacies for 2023. These transactions include Kroger’s merger with Albertsons and CarepathRx’s sale of the BioPlus Specialty Pharmacy to Elevance Health.

We have computed or estimated the figures on a calendar-year basis. The fiscal years for many public retail companies do not correspond to calendar years.

Many companies do not report prescription revenues. We have therefore used various methods and sources to estimate the data.

As noted in the footnotes to the table above, we have made various adjustments to account for the pro forma impact of mergers and acquisitions as well as certain client transitions among the largest PBMs. Year-over-year growth rates were also computed based on the prior year’s pro forma revenues. Pro forma revenues are computed based on the year in which an acquisition was completed.

Unless otherwise noted, the figures for prescription dispensing revenues and profits in our new report (including the table above) exclude COVID-19 vaccinations that were administered in retail pharmacies. This omission permits more appropriate comparisons with previous years’ figures.

Another reminder: prescription dispensing revenues are not equivalent to net spending on outpatient prescription drugs (reported by the Centers for Medicare & Medicaid Services’ National Health Expenditures data) and “non-discounted spending” or “invoice-price spending” (reported by IQVIA).

To complement that broader ranking, we rummaged around our DreamHouse to update our exclusive list of the top 15 pharmacies based on specialty drug dispensing revenues in 2022.

For 2022, pharmacy benefits managers (PBMs) and insurers expanded their dominance over specialty dispensing, though both hospitals and physician practices are growing their share of the market. The top three PBM-owned specialty pharmacies accounted for nearly two-thirds of prescription revenues from pharmacy-dispensed specialty drugs.

As predicted last year, Walgreens’ specialty pharmacy business has collapsed, which led to a slightly less concentrated industry. Read on for additional observations about hospitals, independent pharmacies, the dynamics that created this market structure, and why life in plastic is fantastic.

Pharmacies and Pharmacy Benefit Managers —ranks the largest pharmacies based on estimated revenues from the dispensing of specialty pharmaceuticals. We discuss many of the individual companies listed below in the subsections of Section 3.3. in the full report. Here are five key reflections on the market share data shown above: 1. PBMs still dominate. For 2022, the top three companies—CVS Health, Cigna, and UnitedHealth Group—accounted for nearly two-thirds of prescription revenues from pharmacy-dispensed specialty drugs.

• These businesses are all owned by vertically integrated organizations that also own a PBM.
• The specialty pharmacy business for all three companies experienced double-digit growth for 2022.2.
• Manufacturer and payer strategies have consolidated specialty market share.
• The concentration of specialty dispensing revenues results largely from strategies that plan sponsors and manufacturers use to narrow specialty drug channels.

This has shifted dispensing to the largest specialty pharmacies owned by vertically integrated organizations. As we document in Profiling Manufacturers’ Limited and Exclusive Pharmacy Networks for Specialty Drugs, the specialty pharmacies owned by the two largest PBMs—CVS Health and Express Scripts—had access to more than half of the specialty drugs in limited networks.

What’s more, the share of large employers with a restricted pharmacy network for specialty drugs has grown, from 36% of employers in 2011 to 68% in 2020. For 2021, employers reported reduced their use of restricted specialty pharmacy networks with one specialty pharmacy, likely due to the COVID-19 pandemic.

For 2022, the use of exclusive specialty pharmacy networks rebounded, but remained below its pre-pandemic level.3. Walgreens’ specialty revenues have plummeted. We estimate that specialty dispensing revenues at Walgreens declined by more than 40% in 2022.

1. The revenue figure for Walgreens includes AllianceRx Walgreens Pharmacy (ARxWP), Walgreens’ specialty pharmacy locations inside hospitals, Walgreens’ retail specialty pharmacies, and specialty drugs dispensed within Walgreens retail drugstores.
2. The ARxWP business suffered significant revenue and customer losses during 2022, due largely to the dissolution of the company’s joint venture with Prime Therapeutics.

Most of ARxWP’s lost business shifted to Express Scripts. These lost clients reduced AllianceRx Walgreens Prime revenues by an estimated $9 billion in 2022. We summarize this sad tale in Section 3.3.4. of our new report,4. Providers continue to gain traction.

• Hospitals and health systems are among the fastest-growing participants in the specialty pharmacy market.
• They now account for almost one-quarter of total accredited specialty pharmacies.
• This category has expanded quickly, as hospitals and health systems have opened internal pharmacies.
• Changes in manufacturers’ 340B contract pharmacy policies have accelerated hospitals’ investments in in-house specialty pharmacy operations.

Many self-insured health systems require their employees—the plan’s beneficiaries—to use the specialty pharmacy that the health system owns and operates. (See Fresh Evidence: How Health Systems Steer Prescriptions to Their Own Specialty Pharmacies,) 5.

• Independent specialty pharmacies remain viable.
• The list above includes four independent specialty pharmacies.
• Independent pharmacies remain the largest category of accredited specialty pharmacies, but account for a minority of total specialty dispensing revenues.
• The industry environment remains difficult for independent specialty pharmacies.

Fewer new specialty pharmacies have started up, and the largest independent companies have been acquired. The remaining independent specialty pharmacies are larger and generally well-capitalized. Honorable mention goes to Hy-Vee stores and its Amber Pharmacy.

Most companies do not report prescription revenues from specialty drugs. DCI has therefore used various methods and primary sources to estimate the data. In some cases, the companies provided us with revenue figures.

As noted in the exhibit’s footnotes, DCI has made various adjustments to account for the pro forma impact of mergers and acquisitions as well as certain client transitions among the largest PBMs. Pro forma revenues are computed based on the year in which an acquisition was completed. Year-over-year growth rates were also computed based on the prior year’s pro forma revenues.

Revenues in the “All other retail, mail, long-term care, and specialty pharmacies” category come from the many pharmacies now competing to dispense these expensive therapies. As we discuss in Chapter 3 of the full report, the specialty market’s growth continues to draw many companies into the business of dispensing specialty drugs.

The market size figure excludes estimated revenues from provider-administered specialty drugs billed under a patient’s medical benefit.

Note that specialty drugs’ share of plan sponsors’ pharmacy benefit costs is higher than their share of prescription revenues, because formulary rebates do not impact prescription dispensing revenues.

How can you refill your medication before your trip? How to obtain refills for medication before traveling: The most common way to refill your medication for traveling is to use a vacation override.

• Vacation overrides are done by asking your pharmacy to contact your insurance company.
• Then the insurance company can provide an override that will allow your medication to be filled earlier than anticipated.
• However, vacation overrides depend on your insurance plan (so it can vary from plan to plan).

Most plans will cover a vacation refill at least one time per prescription. If your plan does not, the other option is to pay out of pocket for your prescription. I recommend only paying for as much as you need to get you through your trip and ask your pharmacy about discounts through the pharmacy or sites like GoodRx.

1. One way to limit the chance of needing a vacation refill is to receive your medications in a 3-month supply.
2. Many insurances offer 3-month supplies for maintenance medications.
3. It can lower the number of times you visit the pharmacy and make sure you have your medication when needed.
4. What if I forgot my medication at home and need to refill it while on vacation? This should be a straightforward process if you’re traveling within the U.S.

You can go to the nearest pharmacy and ask the pharmacy to transfer your prescription to have it filled there. You may encounter a similar issue to refilling your medication early; therefore, the pharmacy may need to contact your insurance for a vacation override.

• If you travel often and this happens to you, my recommendation is to use large retail pharmacies because the process of transferring a prescription is generally much easier (e.g., Walgreens in California to Walgreens in New York).
• Larger retail pharmacies are also available in more places and have extended hours compared to an independent pharmacy.

Tips before going to the pharmacy to get vacation refills :

Count how many tablets you have currently and make sure you will need a refill while away on vacation. Some insurance can limit the number of times you can use a vacation override; if you don’t need one, it may be better to save your vacation override for another time. Call your insurance company to know your plan’s policy on vacation refills. Every insurance plan is different, so the more you know, the better prepared you can be for your options. Give your pharmacy at least a one-week notice. If you have many medications sometimes it may take time to get everything ready for pickup. Some medications could be out-of-stock and may take a day or two to receive at the pharmacy. When going to the pharmacy, be prepared to give information such as the dates you are going on vacation, and where you are going, Some insurances may ask for these details to do an override.

Summary Obtaining medication while traveling is not fun and not how you want to spend your time; the best thing to do is be prepared and proactive about your medication refills.

If you’re currently taking a prescription medicine, those foreign trips offer a few new obstacles. It’s essential for travelers to understand how to travel with medicine. Some travel prep means you don’t make it across the world and suddenly run out of something you’re unable to get at the local pharmacy.

The severity of running out of your medication is heavily-dependent on your health condition. For some travelers, missing a dose of their medication is a mild issue. For others, it can be life threatening. First things first, talk to your doctor, Just like you would consult your doctor well ahead of your travel plans, a talk about medicine is also important.

Discuss your medication needs, the length of your trip, and your refills. Along with giving you the appropriate paperwork, a physician can help with larger prescriptions of that medicine. For travelers that are going away for months at a time, the doctor can help with 60 or 90 days worth of medication.

When you speak with your doctor be sure to get a copy of all your medical information, That includes your prescription and/or a letter explaining your condition and what medicines are used to treat it. Keep this information with you while traveling. Make a few hard copies along with a digital copy, just in case something happens to your luggage.

If your insurance doesn’t allow for an early refill or multiple refills at the same time, you may need to pay out of pocket. Travelers could also ask their doctor if there are generic forms of the medications. You may also consider travel insurance for your time abroad as a safety net.

We may not want to consider these nightmare scenarios when traveling, but they do happen. If your bag is stolen with all your medication inside or a trip gets extended and you don’t have enough medication, insurance helps. Just contact those listed in your travel insurance policy and they can act as a guide for your next steps depending on the situation.

Traveler’s insurance can also save some time and hassle on the phone. Rather than spending hours talking to doctors and pharmacies back home, service representatives make the calls. After sorting out the situation, they then advise how to proceed in your current location.

1. It’s always important to plan for the unexpected when traveling.
2. You may miss flights or your luggage may get lost.
3. The hotel might even lose your reservation, leaving you stranded.
4. If possible, bring more than the anticipated amount of medication for your trip.

It will be tempting to save space in your carry on by combining packaging and using pill holders. UHG formed Optum by merging its existing pharmacy and care delivery services into the single Optum brand, comprising three main businesses: OptumHealth, OptumInsight and OptumRx.
