This agreement was made possible as a result of Prime’s desire to expand its service offerings. Any plans that choose to incorporate Express Scripts Pharmacy and Accredo on or after January 1, 2021 have already kicked off the implementation process.” In addition, I had a conversation with Kyle Skiermont, who is Prime’s senior vice president of specialty pharmacy and home delivery services, the day before.

IMPLICATIONS The following is a list of four observations and lessons that may be learned from this new arrangement: 1.
Express Scripts is going to increase its dominance in the specialty and mail-order dispensing markets.
Cigna has risen to the position of the third-largest pharmacy chain in the United States thanks to its Express Scripts operation.

The PBMs’ primary focus should now be on the pharmacy industry. According to our estimates, the mail order and specialty pharmacy dispensing activities contributed more than half of the PBMs’ total gross income in 2019. (For further information on this topic, please refer to Section 11.2.3. of our 2020 Economic Report on U.S.

Pharmacies and Pharmacy Benefit Managers.) The 18 Blue Cross and Blue Shield health plans that have equity holdings in the company use Prime as a pass-through pharmacy benefit manager (PBM) to manage their prescription drug benefits. Additionally, Prime offers PBM services for a total of five other plans in addition to direct employer groups.

It is responsible for the distribution of benefits to more than 30 million individuals.
Because of this, a large portion of Express Scripts’ mail-order and speciality business may go in their direction.

The AllianceRx Walgreens Prime partnership will eventually dissolve. The specialty and postal pharmacy relationship between Walgreens Boots Alliance (WBA) and Prime is currently being tested by new obstacles. Midway through 2016, WBA and Prime Therapeutics made the announcement that they will be becoming AllianceRx Walgreens Prime.

Although it has contracts with (and dispenses for) a number of other PBMs and plans, this joint venture has become the sole mail and specialty pharmacy for the members of Prime Therapeutics’ PBM who get benefits.
It is anticipated that the partnership between Prime and Walgreens would last through 2026.

(In case you were wondering, I was strongly cautioned against using the abbreviation “AWP” while discussing this company.) Due to the fact that the company is considered to be a subsidiary of WBA from a legal standpoint, its financials are included inside the more comprehensive financial reporting of WBA.

Because WBA does not declare its sales for the company separately, the revenue amount that we use for our ranking of the 15 top specialty pharmacies comes from estimates provided by the Drug Channels institute. (For a list of the top 15 specialty pharmacies in 2019, see “The Top 15 Specialty Pharmacies: PBMs Stay on Top.”) In my post from January, I expressed some skepticism over the partnership between Walgreens and Prime: It is not really apparent that the Prime arrangement has actually provided Walgreens with the benefits that were first promised.

Walgreens has experienced lower rates of incremental growth as a result of Prime Therapeutics’ slower expansion compared to that of its larger competitors. In addition, it seems to me that Blue Cross and Blue Shield customers of Prime have been less active in their utilization of limited and exclusive pharmacy networks for speciality pharmaceuticals.

That a direct consequence of this, AllianceRx Walgreens Prime did not enjoy the same surge in business as other PBM-owned specialty pharmacies had.
And with regards to its non-Prime business, AllianceRx Walgreens Prime found itself in a situation that was comparable to that of other independent specialty pharmacies.

Walgreens Pharmaceutical has effectively been kept afloat because to the extremely lucrative 340B contract pharmacy business. Reportedly made public during the Nephron event, Prime disclosed that at least three of Prime’s health plan clients will switch from AllianceRx Walgreens Prime to Express Scripts on January 1, 2021.

It is my guess that other plans will be developed, particularly if Express Scripts presents aggressive pricing in an effort to secure the contract.
The more volume that leaves AllianceRx Walgreen Prime, the more scale it will lose from a business that is already in a weakened position.
A stampede toward the exits might potentially bring about the gradual dissolution of the partnership between Walgreens and Prime before 2026.

It is important to keep in mind that AllianceRx Walgreens Prime was selected in preference to CVS Health in order to handle the specialty pharmacy benefit for the Federal Employee Program because of the cost savings and efficiency it offers (FEP).5.4 million federal employees, retirees, and their families are covered by the three-year FEP agreement that went into effect in January 2018.

If Prime is successful in keeping the business, Express Scripts will come out ahead.
Alex Gourlay, who was serving as the president of Walgreens at the time, stated in January that he was “quite sure that would benefit from that connection in the future.” I seriously doubt that is how they now feel.
Unfortunately, on today’s earnings call, WBA’s management will most likely avoid answering any challenging questions regarding these most recent happenings.3.

Express Scripts is developing an innovative general purchasing organization (GPO) and pharmacy platform. According to my understanding, Express Scripts is constructing its own own in-house negotiation and sourcing platform so that other PBMs may utilize it. Who Is Prime Therapeutics Specialty Pharmacy In 2017, Walgreens entered into a commercial partnership with Express Scripts for the purpose of acquiring generic medications through its Walgreens Boots Alliance Development (WBAD) division. Express Scripts has taken part in this through their Innovative Product Alignment program (IPA).

According to reports, Express Scripts severed its partnership with Walgreens Boots Alliance for the acquisition of generic drugs in the year 2020.

In 2018, Walgreens and Express Scripts came together to establish ValoremRx Specialty Solutions, an independent group buying company with the primary mission of acquiring specialty brand pharmaceuticals. It would appear that this entity is no longer active at this time.

Prime is currently sourcing formulary rebates with brand-name manufacturers through Ascent Health Services, a covert group buying organization (GPO) located in Switzerland that was established by Express Scripts in 2019. As a direct result of this, Ascent is now in charge of rebate negotiations for the pharmacy benefit management (PBM) division of Express Scripts as well as the smaller PBM operated by Kroger and owned by Kroger.

In the future, more smaller PBM rivals could join Ascent (for rebates), as well as the pharmacies owned by Express Scripts (for generic and specialty purchasing). Several of these PBMs do not have their own internal dispensing operations, which contributes to their poor performance in managing net medication costs.

In an effort to reform the medication distribution system in the United States, several large businesses have developed that unite pharmacy benefit managers, specialty pharmacies, insurance companies, and healthcare providers.

As a result of heightened scrutiny from payers, regulators, and politicians, business models for PBMs are undergoing development.
The current management team of Prime is moving quickly to reverse the plan implemented by the previous management team.
Instead of attempting to expand its own pharmacy benefit management (PBM) capabilities, the corporation is growing increasingly reliant on outsourcing to the Express Scripts division of Cigna.

As I hypothesized back in January, the many partnerships that Prime has struck with Express Scripts allow the firm to maintain its alternatives and flexibility. On the other hand, by doing so, they pave the door for Prime to be bought by Cigna or folded into the latter’s operations.
