According to our estimates, the mail order and specialty pharmacy dispensing activities contributed more than half of the PBMs’ total gross income in 2019. Pharmacies and Pharmacy Benefit Managers.) The 18 Blue Cross and Blue Shield health plans that have equity holdings in the company use Prime as a pass-through pharmacy benefit manager (PBM) to manage their prescription drug benefits. Additionally, Prime offers PBM services for a total of five other plans in addition to direct employer groups.

Walgreens has experienced lower rates of incremental growth as a result of Prime Therapeutics’ slower expansion compared to that of its larger competitors. In addition, it seems to me that Blue Cross and Blue Shield customers of Prime have been less active in their utilization of limited and exclusive pharmacy networks for speciality pharmaceuticals.

  1. That a direct consequence of this, AllianceRx Walgreens Prime did not enjoy the same surge in business as other PBM-owned specialty pharmacies had.
  2. And with regards to its non-Prime business, AllianceRx Walgreens Prime found itself in a situation that was comparable to that of other independent specialty pharmacies.

Walgreens Pharmaceutical has effectively been kept afloat because to the extremely lucrative 340B contract pharmacy business. Express Scripts is developing an innovative general purchasing organization (GPO) and pharmacy platform. According to my understanding, Express Scripts is constructing its own own in-house negotiation and sourcing platform so that other PBMs may utilize it. Who Is Prime Therapeutics Specialty Pharmacy In 2017, Walgreens entered into a commercial partnership with Express Scripts for the purpose of acquiring generic medications through its Walgreens Boots Alliance Development (WBAD) division. Express Scripts has taken part in this through their Innovative Product Alignment program (IPA).

In 2018, Walgreens and Express Scripts came together to establish ValoremRx Specialty Solutions, an independent group buying company with the primary mission of acquiring specialty brand pharmaceuticals. It would appear that this entity is no longer active at this time.

Prime is currently sourcing formulary rebates with brand-name manufacturers through Ascent Health Services, a covert group buying organization (GPO) located in Switzerland that was established by Express Scripts in 2019. As a direct result of this, Ascent is now in charge of rebate negotiations for the pharmacy benefit management (PBM) division of Express Scripts as well as the smaller PBM operated by Kroger and owned by Kroger.

In the future, more smaller PBM rivals could join Ascent (for rebates), as well as the pharmacies owned by Express Scripts (for generic and specialty purchasing). Several of these PBMs do not have their own internal dispensing operations, which contributes to their poor performance in managing net medication costs.
