Nearly half of the prescriptions for the drugs that were filled were not followed properly.4 Failing to take drugs as directed can have serious and expensive repercussions; for example, it might result in improper illness management, which can lead to hospitalizations and even fatalities.4 Both visits to emergency rooms and lengths of stays in hospitals rose, with the former increasing by 17% and the latter by 10% annually.5 This was mostly the result of not being able to purchase medication, which is a widespread issue that contributes to noncompliance with drug regimens.

These data serve to further illustrate how noncompliance might result in greater expenses for the healthcare system.5 The fact that non-adherence to medication causes the deaths of roughly a quarter million individuals in the United States each year is an even more frightening statistic.6 All of these expenditures can be avoided entirely if the appropriate counseling and education of the patient is provided, which is primarily the duty of the pharmacist.4 The New England Healthcare Institute (NEHI) estimates that a yearly savings of roughly 290 billion dollars might be realized if patients were educated about their medications and were more compliant with their treatment plans.7 The adherence of patients is a component of pharmacy that has shown to be a persistent source of difficulty.

The issue that has to be answered is, “How can we solve it?” Is it possible for us as pharmacists to genuinely make a difference and enhance a patient’s compliance in taking their meds as prescribed? In a study that was carried out in retrospect during the fall of 2010, participants who had hyperlipidemia were given prescriptions for simvastatin and were split into two groups: the first group received counseling from local community pharmacists, while the second group simply took their medication and went home.

Each patient was asked whether they were available for consultation with the pharmacist, and those patients who declined did so for a variety of reasons, including time restrictions, the fact that a caregiver or someone other than the patient was picking up the prescription, etc.

This involved dividing the total number of days’ worth of medication that was possessed during the treatment period by the total number of days that were spent receiving treatment. Everyone with a value higher than 80 percent was regarded to be compliant with the treatment plan.8 The findings indicated that individuals who received counseling on the advantages of remaining compliant with their medication regimen had a significantly increased likelihood of being compliant (40.9% for the group who received counseling versus 33.7% for those who did not, and the difference was found to be statistically significant using a p0.05).8 Although the MPR and adherence both showed a general decreasing trend over time for both groups, the rate of fall was significantly lower for the intervention group throughout the study.

Following a period of one year of observation, the intervention group had a much higher rate of adherence, as determined by MPR, than the control group did; this difference was statistically significant at p 0.001. In terms of whether or not the patients filled their prescription within the allotted period of time, persistence was also higher for the intervention group in terms of whether or not they filled their prescription on time, with 43.9% filling on time as compared to the 38.2% in the comparative group (p=0.05).8 The findings of this study lend validity to the notion that pharmacists play an important role in the treatment management and adherence of their patients, despite the fact that further research has to be carried out employing bigger populations and a wider range of illness conditions.

More patients may become compliant with their drug regimens if the pharmacist provides them with additional information and counseling, which might result in a reduction in the financial expenses and difficulties that arise from non-adherence.
